Background Check | Training Request for Youth Programs - Departments must submit all requests for background checks and/or Texas State required training courses; list of program personnel must be included using the service request template.

Youth Program | Activity Registration Form - Departments must register, in advance, their program|activity|event|camps by completing this form.

Youth Program Verification Form - Allows the program manager to indicate compliance with the standards for programs involving youth.

Youth in Labs Registration - Provides the Youth Protection Program and the University lab supervisor who sponsors youth with general information pertaining to the youth's educational or volunteer program in the lab.
OPTIONAL FORMS
- Youth Program Insurance Request - Programs must purchase accidental medical insurance for the group through Risk Management Services (RMS) if it cannot be verified that all youth have personal medical coverage.
