Risk Management Services (RMS) is available to help address incidents across campus. Review the incident types below and select the most appropriate option for the issue you need to report. Your submission will be directed to the appropriate department for follow-up.

Incidents reported to RMS are not part of emergency communications to triage the incident while it's still happening. Incidents should only be submitted after the emergency has been addressed and there is no immediate threat to life or health.

Order of operations for fire, serious injury, or immediate threat to life or health:

  • Dial 911 to dispatch emergency services.
  • Contact supervisor, adjacent occupants, and building manager.
  • Dial 940-565-2109 during business hours to speak with someone from our team. After hours, our on-call personnel will be contacted by UNT Police dispatcher.
  • UNT Police non-emergency number is 940-565-3000 and is answered 24 hours a day.

In addition, if you find something that needs to be fixed on campus (ex: a light that is out or a broken fixture) please reach out to Facilities so that they can address it. Contact Work Control to file a service ticket by visiting the Facilities website or calling 940-565-2700.

Risk Management Incident Reporting

The following are for reporting Risk Management related incidents. If you have another incident that isn't listed here, please email us at AskRMS@unt.edu.

For: Faculty, staff and students reporting a spill of hazardous materials on campus. (In emergencies, call 911 first.)

For: Faculty, staff and students reporting exposure to blood or other potentially infectious materials during the course of their duties at UNT.

For: Faculty, staff and students reporting suspected or confirmed foodborne illness from a UNT dining establishment.

For: Faculty, staff and students reporting incidents that occur in a laboratory or involve unsafe lab-related conditions. (In emergencies, call 911 first.)

For: Faculty, staff and students reporting unsafe conditions or incidents involving radiological or laser exposures. (In emergencies, call 911 first.)

For: All employees. This form is to be completed when a State-owned vehicle or low speed unit has been involved in an accident; damaged while parked; or stolen. (In emergencies, call 911 first.)

For: Faculty, staff, and students to report unsafe conditions or incidents that could have resulted in injury, illness, or damage but did not—helping us prevent future accidents.

For: This form is to be filled out when an employee is injured within the course and scope of their duties.
