CONFINED SPACE ENTRY PERMIT

The University of North Texas requires a permit when work is to be performed in a confined space to ensure employees can enter and work in those spaces safely. Work in confined spaces is only approved for short periods of time as they are not designed for continuous occupancy by employees.

A confined space is defined as:

  • Large enough and so configured that an employee can bodily enter and perform assigned work; and
  • Has limited or restricted means for entry or exit (e.g., tanks, vessels, silos, storage bins, hoppers, vaults, and pits are spaces that may have limited means of entry.); and
  • Is not designed for continuous employee occupancy.

Work done in confined spaces must be approved and permitted by submitting a Confined Space Entry Application. If requesting a permit, please do so at least 2 business days in advance.

Once your permit is approved by a Risk Management team member, a copy of the permit and Confined Space Entrant Attendant Log must be posted at the entry site.

*Confined space work that involves hot work will also require a Hot Work Permit.

ELEVATED WORK PERMIT

When working from elevation, such near an unprotected side or edge or from an aerial lift, fall protection and an elevated work permit are required.  Work performed from a properly installed ladder or scaffold does not require a permit. This program is closely aligned with 29 CFR 1926 Subpart M.

Work done from elevated spaces must be approved and permitted by submitting an Elevated Work Permit at least 2 business days in advance.
