Background Check | Training Request for Youth Programs - this form must be submited to request background checks and/or the assignment of the Texas State required training course; list of youth program personnel must be included using the service request template.

Youth Program | Activity Registration Form - the coordinator of every youth program must register, in advance, their activity/event by completing this form.

Youth Program Verification Form - this forms allows the youth program manager/coordinator to verify compliance with the standards for programs involving youth.

Youth in Labs Registration - Form outlines the guidelines for hosting youth at UNT labs. Faculty members and/or lab managers must register all non-UNT minors prior to participating in shadowing or volunteer activities in any laboratory settings. Our department works in partnership with our UNT Office of Research and Innovation to secure final approval.
OPTIONAL FORMS
- Youth Program Insurance Request - Programs must purchase accidental medical insurance for the group through Risk Management Services (RMS) if it cannot be verified that all youth have personal medical coverage.
