CONFINED SPACE ENTRY PERMIT

A permit is required before entering a permit-required confined space at UNT. The permit process helps identify hazards, verify entry requirements have been met, and ensure appropriate controls are in place to protect workers.

A confined space is a space that:

• Is large enough for an employee to bodily enter and perform work

• Has limited or restricted means of entry or exit

• Is not designed for continuous occupancy

Submit a Confined Space Entry Application at least two (2) business days before the planned entry date. To determine whether a space has been identified as a permit-required confined space, please review the UNT Confined Space Inventory.

Once approved, a copy of the permit and the Confined Space Entrant/Attendant Log must be posted at the entry location for the duration of the work. Completed logs must be submitted to Risk Management Services following completion of the entry.

AERIAL LIFT PERMIT

An Aerial Lift Permit is required before operating an aerial lift on campus. The permit process helps ensure appropriate fall protection, equipment inspections, traffic and pedestrian controls, and work zone protections are in place before work begins.

Submit an Aerial Lift Permit Application at least twenty-four (24) hours before the planned work activity.

Workers must comply with all applicable safety requirements, including:

• Use of required fall protection equipment

• Establishment of appropriate barricades and exclusion zones

• Protection of pedestrians and vehicle traffic below and around the work area

• Inspection of equipment before use

• Use of a trained and authorized operator

Work involving aerial lifts should not begin until the permit has been approved and all required safety controls have been implemented.
